Depending on your condition and past medical history, Dr. Jurash will evaluate and determine if an in office appointment or a telehealth appointment is appropriate to begin care. All Sleep Medicine patients are seen in office.

Insurance
At our office, we hold your health and wellbeing in the highest regard. Insurance companies tend to dictate treatment based on cost cutting measures. These treatment recommendations are oftentimes not in line with the best treatment outcomes for patients. For these reasons, we have opted to not work with medical or dental insurance companies.

At the end of your visit, we will provide you with a superbill, a document that you can submit to your insurance company to seek reimbursement. If you wish to proactively explore your coverage, we are more than willing to supply you with the billing codes in advance. Many of our services qualify for HSA expense reimbursements.

Sleep Apnea patients: Our office is happy to submit an insurance claim for coverage of an oral appliance for Sleep Apnea. To streamline this process, we kindly request that you bring a copy of your sleep study, a prescription for an oral appliance from your physician, a copy of your insurance card, and your driver's license to your initial appointment.
Referrals
If you wish to refer a patient to The Center, please utilize the contact form above. Alternatively, you can print out a referral PDF and fax or email it to our office, along with any imaging of the jaw joints, such as panographs or CBCT scans. Our team understands the importance of seamless communication, and we are committed to providing a hassle-free referral process for our valued patients.
